Statutory Sick Pay

Statutory Sick Pay (SSP) is money paid by employers to their employees who satisfy the conditions for payments when they are ill and unable to work.
Please fill in this form when you have been sick for 4 days or more in a row.
Covid-19 - Record of Vaccination
If you require a record of your vaccination, you can request this from NHSinform.
Do not contact your GP practice about your coronavirus vaccination status. GP's cannot provide letters showing your coronavirus vaccination status. Your letter can take 14 days so if you require a vaccination status letter for overseas travel, make sure you request this in plenty of time.
PRESCRIBING DIAZEPAM FOR FEAR OF FLYING
The practice no longer provides this service to patients.
The practice no longer issues prescriptions of Diazepam or other sedatives for fear of flying. Please see the documents section of the website for further information and advice.
